Congratulations to the Officers and Crew Who Served on These Vessels During Q2-2016!

There is a very strong correlation between good hazard reporting and safety related incidents onboard. Vessels that promote hazard reporting create an atmosphere conducive to an open reporting culture generating amongst the complement a constant vigilance to their surroundings. This identification of hazards creates a safer working environment onboard limiting the number of incidents. Conversely, on ships, where hazard and near miss reporting is less than satisfactory, the personnel remain unaware of the hazards lurking around them and such complacence leads to injuries and accidents. Starting January of this year, the hazard reporting target for Teekay Tankers was raised to a mandatory minimum of 30 hazards per month for each vessel. In order to promote a robust hazard reporting culture onboard, we had introduced a quarterly award to be given to one ship from each of the four teams that reported the highest number of hazards during the preceding three months. It gives us great pleasure to announce and recognize the proud recipients of this hazard reporting awards for the first quarter of 2016. The following vessels have reported the highest number of hazards in their teams in Q2-2016:

Team Aquila: Emerald Spirit – 160 hazards
Team Centaurus: Leyte Spirit – 358 hazards (A special congratulations to the Leyte Spirit for getting this award for a second quarter in a row.)
Team Lynx: Esther Spirit- 229 Hazards
Team Orion: Ashkini Spirit- 317 Hazards

Congratulations to the officers and crew who served on these vessels during the second quarter of 2016. They deserve our applause. So as to achieve our Operational Leadership objectives of getting everyone home safely and delivering flawless customer service, we all need strictly follow procedures and diligently use all the risk management tools available. The best way to begin is by initiating a firm practice of persistently reporting hazards, which in the long run are the root cause of all injuries, incidents and accidents. We all enjoy working in a safe work environment; one that gets us home safely to our family and friends and makes us proud of the Teekay banner that we sail under. Therefore, let us continue in our quest to report all the hazards that we observe and identify, so that we have fewer personal and operational incidents. Once again, well done to the ships that are receiving this award for Q2-2016 and we encourage the others vessels to follow the fine example set by these four vessels to step up their hazard reporting.
